ABSTRACT

Objective: To compare the efficacy and safety of oral misoprostol (Angusta®) versus vaginal dinoprostone (Propess®/Prostin®) for induction of labor at term. Methodology: A retrospective descriptive and analytical cohort study conducted over 5 years (January 2020 to December 2024) at the EpiCura hospital, Ath. Eligible patients had term monofoetal pregnancy, cephalic presentation with an unfavorable cervix requiring induction of labor. Data were collected and analyzed using Excel 2016 and IBM SPSS 29. Quantitative variables were compared using the Student’s t-test, while qualitative variables were compared using the chi-square test, with a significance threshold of p < 0.05. Results: Of 3,669 deliveries, 766 induced, representing an induction rate of 20.8%. A total of 684 patients were analyzed: 244 induced with oral misoprostol and 440 with vaginal dinoprostone. Groups were comparable in terms of maternal age, BMI, gravidity, and Bishop score. However, significant differences were observed regarding parity and gestational age, with a higher proportion of primiparous women and prolonged pregnancies in the Propess®/Prostin® group. Prolonged pregnancy was the main indication for induction in both groups. The rate of vaginal delivery within 24 hours was significantly higher with oral misoprostol compared to vaginal dinoprostone (92.6% vs. 70.2%). Cesarean rate, oxytocin use, maternal tolerance, and neonatal outcomes were similar. Conclusion: Oral misoprostol is more effective than vaginal dinoprostone for labor induction at term, with comparable maternal and neonatal safety.
